Qualifications

✓Strong clinical nursing knowledge with the ability to perform patient assessments, identify changes in condition, and appropriately triage clinical concerns.
✓Ability to obtain, monitor, and accurately document vital signs, patient histories, and other clinical information.
✓Strong interpersonal, verbal, and written communication skills with the ability to effectively engage patients, families, providers, and colleagues.
✓Ability to collaborate effectively in a team-based healthcare environment while maintaining professionalism, confidentiality, and a patient-centered approach to care.
✓Strong organizational skills, proficiency in medical terminology, and the ability to prioritize multiple responsibilities while adhering to regulatory, professional, and organizational standards.

Experience supporting care coordination activities, including referrals, prior authorizations, prescription refills, and patient follow-up. preferred
Ability to provide patient education and promote positive patient experiences through effective communication and engagement preferred
Experience supporting staff training, onboarding, mentoring, and clinical workflow improvement initiatives preferred
Demonstrated commitment to quality improvement, teamwork, and the delivery of high-quality, patient-centered care preferred
