Licensure
✓Current licensure as a Registered Nurse, Physical Therapist or Occupational Therapist in the State of Maine required.
Certifications
✓Current Basic Life Support (BLS) certification is required or must be obtained within 30 days of start date.
Case Management certification (CCM, ACM-RN, or equivalent) preferred.
Education
✓Associate’s degree in appropriate clinical field required; bachelor’s degree preferred.
Qualifications
✓Three years’ direct clinical experience required.
✓Knowledge of post-acute levels of care, including skilled nursing, inpatient rehabilitation, long-term acute care, home health, hospice, and related services.
✓Knowledge of healthcare reimbursement and coverage requirements affecting post-acute services.
✓Ability to review and interpret clinical documentation and patient care needs.
✓Proficiency with electronic medical records and referral management systems.
Experience working in a skilled nursing facility or post-acute entity, discharge planning, utilization management, admission management, or post-acute care coordination preferred.
Experience in a healthcare setting involving care transitions across multiple levels of care preferred.
Knowledge of value-based care models and population health strategies. Knowledge of post-acute provider networks and community resources. preferred
