40 hours per week, day shift 9am-6pm, occasional Saturday. This position is on-site.
Coordinates and facilitates patient admissions and transfers according to patient acuity, bed availability, and required services. Collaborates with nursing staff and multidisciplinary team members to place patients in the appropriate level of care and coordinates as liaison between physicians and specialty consultants. Initiates care management process including pre-registration, insurance verification, and review of medical reports from referring hospitals or physician offices.
Stays informed of managed care changes and communicates procedural and financial impacts to admitting physicians during the scheduling process. Maintains comprehensive documentation of all communications with physicians and referral sources for legal, medical, and quality improvement purposes. Develops and maintains relationships with referral sources and admission decision makers to ensure efficient patient placement and access.
Qualifications Licensure / Certification / Registration: - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.
