Position is PRN, weekday and weekend availaibiliy needed, 8am-4:30pm. Must be willing to train during weekday hours. As the Per Diem Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient.
Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care. Primary
Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition Documents problems, appropriate goals, interventions, and patient/family response to hospice care Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
Required
Current unrestricted RN licensure in the state of practice 1+ years of clinical experience Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation Preferred Qualification: Current CPR Certification or ability to complete within 90 days of hire Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.

The hourly pay for this role will range from $37.40 to $56.10 per hour based on full-time employment.
