Job Summary and Qualifications

Are you seeking a nursing role that goes beyond the traditional Progressive Care Unit experience? Our specialized Progressive Care Transplant & Advanced Heart Failure Unit provides highly complex care for liver transplant, kidney transplant, and Left Ventricular Assist Device (LVAD) patients throughout their recovery and long-term management journey. As a Registered Nurse on this unique unit, you will care for a diverse population of medically complex patients requiring advanced clinical expertise, critical thinking, and multidisciplinary collaboration.

This role offers the opportunity to work alongside transplant surgeons, transplant coordinators, advanced practice providers, cardiologists, and critical care teams to deliver exceptional, life-changing care. Our nurses develop specialized knowledge in transplant medicine, immunosuppressive therapies, advanced hemodynamic monitoring, mechanical circulatory support, and complex patient education while caring for patients recovering from some of the most sophisticated procedures in modern medicine.

What Makes This Unit Unique: Specialized care for liver and kidney transplant recipients. Dedicated LVAD patient population with advanced mechanical circulatory support needs. Exposure to complex post-surgical and chronic disease management.

Extensive collaboration with nationally recognized transplant and heart failure teams. High-acuity patient population requiring advanced clinical judgment and critical thinking skills. Opportunities for advanced education, specialty certification, and professional development.

Key

Provide comprehensive nursing care for liver transplant, kidney transplant, and LVAD patients in a progressive care setting. Monitor and assess patients for signs of transplant rejection, infection, hemodynamic instability, and post-operative complications. Manage complex medication regimens, including immunosuppressive therapies, anticoagulation, and vasoactive medications as appropriate.

Perform advanced assessments and intervene promptly when changes in patient condition occur. Care for patients requiring specialized monitoring and management of LVAD devices. Collaborate with multidisciplinary teams including transplant physicians, surgeons, pharmacists, dietitians, physical therapists, and case managers.

Educate patients and families regarding transplant recovery, medication compliance, LVAD management, lifestyle modifications, and discharge planning. Support patients through the physical and emotional challenges associated with organ transplantation and advanced heart failure. Maintain accurate and timely documentation utilizing the electronic medical record.

Participate in quality improvement initiatives and evidence-based practice activities.
