More about our team As an RN Case Manager at Canyon Vista Medical Center , you will play a vital role in guiding patients through their healthcare journey while serving as a key connection between patients, families, physicians, and insurance providers. Our case management team works collaboratively across the Medical-Surgical, Telemetry, and Intensive Care units to coordinate care, advocate for patients, and ensure safe, successful transitions from hospitalization to home or the next level of care.
Based on Canyon Vista Medical Center's Case Management program, nurse case managers are deeply involved in discharge planning, care coordination, resource management, and daily interdisciplinary collaboration to support quality patient outcome. How you’ll contribute The RN Case Manager monitors and manages patient care to promote continuity of care, optimal outcomes, patient satisfaction, cost efficiency, and regulatory compliance.
Collaborates with the multidisciplinary team, payors, and community resources; leads discharge planning and utilization review activities. Reports to the Director or Manager of Department.
Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge. Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors. Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
Performs chart reviews to identify and address issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement. Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression. Accurately documents case management activities, plans, and outcomes.
Participates in performance improvement and required continuing education. Supports department operations by working alternate shifts/units as needed; may be assigned on-call. Performs other duties as assigned.

What we’re looking for Applicants should have a current Arizona or compact state RN license and possess an associate’s degree from an accredited nursing school.
Additional requirements include: Associate’s Degree in Nursing required, Bachelor's or Master's Degree preferred. Must have an active Arizona or Compact RN License. 1 year of experience in Case Management/Utilization management preferred. 5 years of experience in an Acute Care Setting preferred. Discharge planning experience preferred.
