About the role

The Nurse Liaison plays a critical role in ensuring the timely and seamless transition of infusion patients from hospital to home. Serving as the first point of contact for new home health referrals, the Liaison is responsible for clinical coordination, reviewing documentation to ensure appropriateness and readiness for home infusion services, and facilitating effective communication among care teams.
