Be the Best Hospice RN You Can BeIf you meet these qualifications, we would love to meet you:Associates degree, preferredRegistered Nurse license required in state(s) practicingCurrent and valid CPR certification, preferred – required in GeorgiaMinimum one year of experience, preferred – required in CaliforniaPrevious experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursingResponsibilities:Conducts comprehensive nursing assessments and develops individualized care plans in coordination with the interdisciplinary team (IDG), addressing physical, emotional, social, and spiritual needsMaintains timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirementsCoordinates care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of careProvides education and support to patients and caregivers, and responds to urgent needs during on-call or admissions assignmentsSupervises LPNs, LVNs, and hospice aides as needed, and participates in IDG meetings, patient conferences, and ongoing clinical educationSupports pre-hospice services by evaluating patient status via phone, telehealth, or in-person visits and identifying needs for additional care or servicesOur Investment in YouCaring for others starts with caring for you.
