Provides comprehensive nursing care and care coordination for patients with HIV/AIDS, Hepatitis, STI management, and other infectious disease conditions in an outpatient setting. Serves as a primary clinical resource for patient triage, symptom assessment, medication management, and treatment adherence support. Coordinates patient care across multidisciplinary teams including providers, pharmacists, social workers, case managers, and community partners.
Reviews laboratory results and diagnostic testing, communicates provider recommendations, and ensures timely follow-up. Administers medications, immunizations, and treatments as ordered. Provides patient education related to disease prevention, medication compliance, health maintenance, and self-management.
Assists with population health initiatives, quality measures, and grant-related requirements. Maintains accurate documentation and ensures compliance with regulatory and organizational standards. Participates in quality improvement activities and supports clinic workflow and operational efficiency.
Minimum Requirements American Heart Association Basic Life Support (BLS) Certification Current license as a RN in the state of North Carolina. Demonstrates competencies in nursing practice. Basic computer skills with excellent verbal and written communication skills.
