We’re Looking For: Completion of an accredited or approved program in nursing Registered Nurse license issued by the state in which the teammate practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association One year of Nursing experience needed
Providing education and support to individuals and families. Coordinating care and connecting families with community resources. Assisting families with accessing healthcare services and follow-up appointments.
Supporting weight management and wellness plans. Providing health education. Collaborating with physicians, dietitians, social workers, and other team members.
Monitoring patient progress and coordinating care. Demonstrates strong nursing practice through comprehensive patient assessment, care coordination, evidence-based education, and proactive management of patient and family needs. Effectively collaborates with interdisciplinary teams to improve health outcomes, remove barriers to care, and support preventive health initiatives.
Consistently promotes patient safety, quality, and engagement while serving as a trusted resource for patients, families, and team members.
