You’ll need
Licensure
✓New York State License with current registration as a registered nurse.
✓For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required.
✓For NPs who bill for professional services, National Board Certification is required regardless of hire date.
✓The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee.
✓Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required
Certifications
✓Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate.
✓State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows:
✓Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination.
✓Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges
Qualifications
✓Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course).
✓Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner.
✓Previous progressive experience relevant to the area of clinical practice required.
✓National certification in a primary field (eg Adult, Gerontology, Family, Acute Adult, etc)
✓3+ years of palliative care/geriatrics experience
✓Experience working in interdisciplinary care teams
Advanced Cardiac Life Support (ACLS)- ACLS recommended/preferred
Hospice and Palliative Certification preferred
Home care, hospice or palliative care experience preferred
Advanced Care Planning experience preferred
Spanish speaking proficiency/fluency preferred

About the role
Palliative Care at Home, part of Mount Sinai at Home, provides home based palliative symptom assessment, treatment and ongoing goals of care discussions for patients and families facing advanced illness. The full clinical team is comprised of CHWs, NPs, nurses, social workers and physicians.
