The RN Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the teams in the ambulatory setting. The nurse will perform medical record review, follow developed protocols, determine appropriate patient scheduling, assist with monitoring diagnostic results and patient care progress/processes as requested by providers, and participate in quality reviews.
