Our client is seeking a dedicated and proactive Registered Nurse - Assisted Living Facility Nursing Case Manager to join this supportive team. A Slower, More Personal Pace: The residents are generally stable, requiring skilled nursing care, but rarely critical or emergency interventions. This allows you to truly focus on proactive health management and building meaningful, long-term relationships.

Perform thorough assessments to understand each resident's unique needs. Develop, implement, and evaluate individualized care plans. Administer medications and treatments with precision and care.

Consistent daytime hours: 7:00 a.m. to 5:30 p.m.
Compassionate, empathetic, and patient-centered. An excellent communicator, capable of building rapport with residents, families, and staff. Proactive in identifying resident needs and developing solutions.
Skilled in clinical assessment and care plan development. Holds an active Registered Nurse (RN) license in New York.
