Registered Nurse RN Case Manager, Home Health $5,000 Sign-On Bonus Ravenna, Streetsboro, Tallmadge and Stow, Ohio Make meaningful connections with your patients, one home visit at a time. Elara Caring is hiring an RN Case Manager to care for patients throughout Summit and Portage counties, including Ravenna, Streetsboro, Tallmadge, Stow, and surrounding communities. Use your nursing skills to help patients stay safe and comfortable at home.
You’ll have flexibility to organize your visits and support from a close-knit clinical team. New to home health? Bring at least one year of nursing experience and a willingness to learn.

Tuition reimbursement for full-time staff and continuing education opportunities. Pet insurance. What You’ll Do Assess patients’ needs and develop individualized care plans.
Manage your caseload and provide skilled nursing care in patients’ homes. Educate patients and caregivers about medications, treatments, and managing their health. Monitor progress and update care plans as patients’ needs change.
Communicate changes and concerns to physicians and your clinical team. Maintain patient confidentiality and complete timely, accurate documentation. What You’ll Need Current, unrestricted state RN licensee 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area.
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations. Must be able and willing to travel 50% Associates Degree or Bachelor’s Degree in Nursing is preferred Previous Home Health or Hospice Experience is preferred Previous experience with HomeCare HomeBase is preferred Where You’ll Work and Your Schedule Provide home health care in Summit and Portage counties , including Ravenna, Streetsboro, Tallmadge and Stow .
