You’ll need
Licensure
✓Current and valid California RN License (required).
Certifications
Certified Case Manager (CCM) or Accredited Case Manager (ACM) (preferred).
Education
✓Associate's degree in Nursing (required).
Bachelor's degree in Nursing (preferred).
Qualifications
✓Two (2) years of nursing experience in an acute care setting (required).
One (1) year of Case Management experience (preferred).
Experience with Utilization Review (strongly preferred).

What you’ll do
Perform evaluations and assessmentsPerform evaluations and assessments within the established timeframes
Detail appropriate reviews forDetail appropriate reviews for assigned patients using our utilization review tool
Provide telephonic reviews forProvide telephonic reviews for identified contracted/private patients and collaborate with on-site and/or outside reviewers
Keep patients informed ofKeep patients informed of progress and provide information related to disease progression
Collaborate with discharge plannersCollaborate with discharge planners to make orders and arrange for home care equipment and healthcare needs
