RN Case Manager, Inpatient

The Inpatient RN Case Manager works with the interdisciplinary team to coordinate the care of the patients focusing on what the patient will need post hospitalization. They will assess, evaluate and coordinate a safe discharge plan with the patients/families’ goals of care as their guide. The goal of the RN case manager is to transition the patients to the next level of care in a timely and safe way with the wishes of the patients/families at the forefront of those transitions.

This professional is responsible for maintaining very effective inter-professional collaboration across the clinical teams involved in patient care, as central to the success of the work.
