Job Summary and Qualifications We are seeking a dynamic Case Management RN to join our team . We need a compassionate and reliable RN to facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management . In this role you will be required to coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization .

IN THIS ROLE: Performs a comprehensive assessment of psychosocial and medical needs of patients Advocate s f or patients while assuming a leadership role with the interdisciplinary team Coordinates plan of care Acts as a liaison through effective a nd professional communications with physicians, patients, families , hospital staff, and outside agencies Performs utilization management reviews and communicates i nformation to third party payors Facilitates patient throughput with an ongoing focus on quality and efficiency
Florida RN license and/or approved multistate RN license required . Compact license holders have 60 days to obtain the Florida license after relocation.
