Job Summary and Qualifications As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. The goal is to give an unmatched patient experience and attain optimal clinical outcomes. What will you do in this role: Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect. Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation. Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge. Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge. Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc. Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions. Demonstrates a time-conscious awareness and ability to prioritize. Distinguishes between low and high-priority activities.
