Provides for individualized case management for those patients who have complex medical or catastrophic conditions which have precipitated the need for extensive and possible non-traditional medical support. Review of intake referrals, ICD-9 coding, CPT coding to ensure full reimbursement from payor. Process referral and additional orders for authorization.
Review for clinical appropriateness and safety for home care. Responsible for obtaining authorization from MCO payor. Promotes evidence-based clinical care that exemplifies the pursuit of clinical excellence.
Home Health RN
RN (Registered Nurse) License Driver's License State of Florida (within 30 days of hire) Required Associate's or Technical Diploma-Nursing Preferred Case Manager experience This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA).
