Responsibilities As the RN Navigator, you will serve as a central point of coordination for patients with traumatic brain injury and complex epilepsy, ensuring timely access to specialized neurological care and reducing barriers throughout the treatment journey. Working closely with physicians, advanced practice providers, rehabilitation services and support teams — you will facilitate care transitions, manage follow-up needs and support patients with complex clinical and psychosocial challenges.
You will leverage tracking and navigation processes to promote continuity of care, monitor patient progression, and help ensure no patient is lost to follow-up. Additionally, you will contribute to the growth and success of the Neuroscience Institute by improving care coordination workflows, enhancing access to services and supporting quality outcomes for a highly specialized patient population. Qualifications Education & Experience High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required 3 years of relevant experience in Neuroscience or case management required Licenses and Certifications RN - Current and valid Registered Nurse license to work within the state of California required BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degree required Disclaimers •
