In this role, you’ll use your clinical skills to provide skilled, short-term acute care to patients one at a time, giving you the chance to build trusted relationships and make a real difference in their lives. You’ll assess patient information such as vital signs, weight, and health-related questions, then use your judgment to determine when follow-up or medical intervention is needed.
You’ll also connect with providers as needed, offer patient education and coaching, and help patients feel informed, supported, and cared for throughout their journey. By working closely with a multidisciplinary team and community resources, you’ll help ensure each patient receives thoughtful, coordinated care. This position also offers day shift hours, mileage reimbursement and/or company vehicle use, and generous continuing education opportunities to support your growth. ✅
Wisconsin RN license is required . Previous RN experience preferred. Valid Wisconsin Driver's License in good standing and liability auto insurance required .
