Qualifications
✓Minimum of five (5) years of nursing, discharge planning, home health, critical care, long-term care, care management, or related clinical experience.
✓Demonstrated success building relationships with healthcare professionals and referral sources.
✓Strong communication, presentation, and interpersonal skills.
✓Ability to work independently, manage a large geographic territory, and prioritize multiple responsibilities.
✓Strong clinical assessment and decision-making skills.
✓Proficiency with Microsoft Office applications and technology platforms.
✓Ability to travel extensively throughout New England, Primarily throughout Boston and Worcester weekly, and to RI, NH, and NY on a monthly basis.
Previous experience in critical care, acute care, long-term care, discharge planning, case management, or transitional care strongly preferred.
