You’ll need
Education
✓Associate degree required
Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
Completion of a Community Health Worker training program (such as Partners in Care) preferred.
Qualifications
✓Strong interpersonal, communication, and problem-solving skills.
✓Ability to work independently in community settings with limited supervision.
Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
Experience working in rural or underserved communities. preferred
Knowledge of social determinants of health and community-based resources. preferred
Ability to complete approved Community Health Worker training during the first year of employment preferred.
