• The Social Services Worker
• MSW is responsible for working with the interdisciplinary team to affect overall care coordination. The Master’s
• Level Social Worker provides comprehensive behavioral health and psychosocial services to patients receiving care in outpatient hospital clinics. The Social Worker collaborates with physicians, advanced practice providers, nurses, therapists, case managers, and other members of the interdisciplinary care team to address patients’ emotional, behavioral, social, and practical needs. This position provides assessment, brief intervention, counseling, care coordination, resource linkage, crisis intervention, and discharge/follow-up support as appropriate to the outpatient setting. Essential Job Duties and

1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served for the following age groups: ☒ Neonate (birth – 28 days) ☒ Infant (29 days – less than 1 year) ☒ Pediatric/Child (1 year – 12 years) ☒ Adolescent (12 years – 18 years) ☒ Adult (18 years – 65 years) ☒ Geriatric (over 65 years) Incorporates cultural considerations in the provisions of care. Knowledge of growth and developmental stages.

Considers life changes/effects on health beliefs and behaviors. Provides necessary safety measures. Provides information and involves family/caregiver in decision making. 2.

Performs all responsibilities/duties required by the outpatient hospital clinics as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes, but is not limited to, the age of the patient served. Complete comprehensive psychosocial assessments to identify emotional, behavioral, social, financial, family, and environmental factors affecting the patient’s health and treatment.

Facilitate referrals to behavioral health providers, community agencies, support groups, financial assistance programs, housing resources, transportation services, substance use treatment, and other appropriate resources. Assist patients with navigating complex healthcare systems and accessing needed services. Maintain communication with community providers and other members of the patient’s care team, consistent with patient consent and applicable regulations.

Participate in interdisciplinary rounds, case conferences, and clinic meetings as appropriate. Collaborate with medical and behavioral health providers to develop coordinated plans of care. Provide consultation regarding psychosocial concerns, community resources, safety issues, and behavioral health needs.

Advocate for patient-centered care and equitable access to services. Complete timely, accurate, and clinically appropriate documentation in the electronic medical record. Document assessments, interventions, care coordination, referrals, and follow-up plans according to hospital and departmental standards.

Participate in quality improvement initiatives and activities designed to improve patient outcomes and the delivery of outpatient services. Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

EDUCATION (Minimum Requirements for Position): MSW Master’s Degree required. Social Services Specialist- Bachelors Degree required EDUCATION FIELD OF STUDY (If Applicable): Mental Health related field required.

CERTIFICATES, LICENSES, REGISTRATIONS: MSW Master of Social Work License required. MSW/Social Services Specialist Non-Violent Crisis Intervention program obtained within six full months from date of hire (Travelers-required upon hire) Previous Experience

Previous clinical experience preferred. Skills/Knowledge
