About the role

Duties Description Capital District Psychiatric Center (CDPC) is recruiting for Social Work Assistant 1 or Social Work Assistant Trainee to provide case management and service coordination activities for individuals with mental illness. Candidates from outside or within State Service can apply. As a Social Work Assistant 1 at CDPC, you will work with an interdisciplinary team and duties will include:
Interviewing individuals and their families to gather background information
Preparing case histories and determining eligibility for services
Advocating and assisting individuals in identifying and obtaining services
