The Social Worker 2 role within the UH Rainbow Babies & Children’s Hospital offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. Serves a resource to others in work-team and department.

• Provides psychosocial assessment / intervention to assist patients/families (including clinically complex)
• Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
• Develops plans with attention to individual patient and family goals
• Provides linkage to community resources and UH network services to facilitate optimal care Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance/ consultation to peers/team regarding routine psychosocial issues and /or discharge planning issues. Provides supervision to social work interns if LISW eligible (completion of supervision hours). Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Licenses and Certifications Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) (Preferred within 1 Year) Position Details/Preferred Qualifications At least one year of recent experience is strongly preferred. This position provides social work services to the Cranial Facial clinic. The Social Worker is responsible for psychosocial assessments, supportive counseling services, providing resources, and crisis intervention, as this patient population faces many challenges. The schedule is Mon/Tues/Thurs/Fri: 8:30am-4:00pm, and Wed: 9:30am-6:00pm for a total of 36 hours per week.
