✓Theoretical and clinical knowledge regarding health care delivery (Required proficiency)

✓Communicates effectively with persons of various backgrounds (Required proficiency)
Hospital/Health Agency experience. (Preferred)
Post-Master’s experience is strongly preferred.
Previous work in oncology or chronic diseases is also preferred.
This is an outpatient role within the Seidman Cancer Center in Cleveland. preferred
This position supports all patients with sickle cell and all benign hematology diagnoses. preferred
This includes assisting with housing, transportation, utility assistance, insurance, mental health and substance use issues, and many other psycho-social barriers to care, for hundreds of patients every year. preferred
This social worker is also responsible for the patient transitions between pediatric and adult sickle cell services. preferred
They also help to prevent hospital readmissions and decrease missed appointments. preferred
