Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. Conducts in-depth assessments/reassessments covering psycho-social, rehabilitation and environmental concerns Refers clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional level Develops the care plan, implement the services and conduct follow-up and monitoring of the plan Identifies and develops support systems for the client Consults with the Health Practitioner Performs case recording and reporting Job Requirements Required Bachelors Other in social work, psychology, counseling, rehabilitation, gerontology or sociology One (1) year experience working with the elderly and individuals with disabilities Where You'll Work Dignity Health Connected Living has been serving seniors in Shasta County since 1968.
