This is a full time, 40 hour per week position with hours primarily falling Monday – Friday 8:00 am – 5:00 pm. We are seeking a Social Worker to: Utilize both clinical and community-based interventions to assist a diverse patient population facing complex medical and psychosocial needs. Address challenges and barriers that affect patient engagement and help improve self-management of their illness within a medical case management framework . Work with patients over time to support adjustment and long-term health.
