Social Worker - Home Health (Days) (Part-Time)

Responsible for assessment of all psychosocial factors and participation in treatment planning of patients/families. Required Knowledge & Skills

Bachelor's degree

One year of related experience. Requires a working knowledge of standard practices and procedures. Licenses and Certifications * Driver's License Qualifications * Bachelors Degree in Social Work required. * Experience - minimum of one year in hospital or medical setting. * Ability to deal comfortably with people in emotional need in a competent, calm manner. * Proficiency in communication skills at all levels. * Proficiency in observation, assessment and problem-solving skills. * Demonstrates flexibility, initiative and organization in performance. * Ability to work independently. *CPR BLS certification or must be obtained within 30 days of hire *Degree in social work and currently licensed or pursuing licensure as a social worker in the state of GA preferred. *One year of home health experience preferred
