Qualifications
✓Provide evidenced-based treatment for patients at CDT.
✓Provide group, Family-Based Treatment and individual treatment.
✓Provide therapeutic support.
✓Program therapists develop treatment plans, monitor and document progress and facilitate discharge planning to meet compliance and billing requirements and successfully return the patient to community resources.
✓Program therapists work within a multidisciplinary team of therapists, medical providers and families to ensure comprehensive treatment.
✓Participate in weekly clinical team meetings with multi-disciplinary team of medical providers and therapists. .

About the role

Responsible for providing therapy, case management services and linkages to community agencies to meet patients' social and emotional needs. Identifies and assesses client patient cases and anticipates their needs related to illness, hospitalization, finances, education and home or community services. Serves as a liaison between health care facilities, community resources and government agencies.
