Senior Social Workers in the Primary Care at Home program play a critical role in supporting patients and families by identifying social, emotional, and environmental needs and connecting them with appropriate resources and services. This role provides a full spectrum of social work and discharge planning services, delivering comprehensive case management for patients with complex medical, geriatric, or behavioral health needs.
As a key member of the care team, the Senior Social Worker helps ensure patients receive coordinated, compassionate support that promotes stability, independence, and overall well-being. Position
Master’s degree in social work (MSW) from an accredited graduate school of Social Work Minimum of 1-2 years of related work experience preferred.
