Job Summary The Housing Coordinator is a Certified Social Worker ( CSW ) responsible for delivering intensive clinical case management alongside housing program coordination for Ryan White clients at the Bluegrass Care Clinic. This position blends the housing assessment, placement, Emergency Hotel Program, and Emergency Financial Assistance functions of the Housing Program with the psychosocial, care engagement, and eligibility maintenance functions typically carried out by Medical Case Management staff.
The Housing Coordinator conducts psychosocial and housing assessments, develops individualized Housing Service Plans, coordinates emergency hotel stays for patients in immediate housing crisis and works closely with Medical Case Managers and the care team to ensure clients remain engaged in medical care and up to date on Ryan White, KADAP , Medicaid, and other eligibility requirements. This role requires clinical judgment beyond routine housing coordination, including crisis intervention, trauma-informed engagement strategies, and coordination of care for clients with complex medical, mental health, and substance use needs.
The position maintains accurate documentation in CAREWare, participates in the Basic Needs Team, and collaborates with community partners to expand housing resources for a highly vulnerable client population. No Preferred Education/Experience MSW with three or more years of case management experience, preferably in a medical or behavioral health setting. Experience working with individuals living with HIV / AIDS and/or unstably housed populations strongly preferred.
