Specialist Case Management HIV

Provides education, advocacy and support to patients and families.

Provides case management and coordination for patients and/or families needing a variety of educational, social, and advocacy services. Researches, develops, and delivers educational materials related to patient and family support based on learning styles and needs of populations served. Provides assistance to patients in developing support systems, linking patients and families to community resources as appropriate.

Works collaboratively with staff, payers, and external resources to identify needs, cultivate relationships with internal/external resources, and enhance clinical outcomes. shown represents the base pay range for this position. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
