Document patient care journeys with precision to maintain accurate records that drive clinical excellence and continuous performance improvement. Support seamless care transitions by developing comprehensive discharge plans and connecting patients with the vital community resources they need to thrive after therapy. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
