You’ll need
Licensure
✓Speech-Language Pathology License in good standing in the state(s) of practice (current or temporary) required .
Certifications
✓Iowa Department of Health and Human Services (IADHHS)
✓Illinois Department of Financial and Professional Regulations (IDFPR)
✓Wisconsin Department of Safety & Professional Services
✓Must possess and maintain current BLS certification.
✓Certificate of Clinical Competence through the American Speech and Hearing Association (ASHA)
✓IA Dependent Adult Abuse Mandatory Training in state(s) where providing care required .
✓I L Dependent Adult Abuse Mandatory Training in state(s) where providing care required .
Education
✓Successful completion of a Master’s Level program accredited by the American Speech & Hearing Associations, Applicants in their Clinical Fellowship Year considered.
Qualifications
✓One year of clinical experience required if it’s with UnityPoint at Home.
✓IA Child Abuse Mandatory Reporter Training in state(s) where providing care required .
✓I L Child Abuse Mandatory Reporter Training in state(s) where providing care required .
Prior work as an SLP in practice area preferred.
