The Substance Use Disorder Clinical Pharmacist Specialist is responsible for delivering appropriate clinical care, supporting patients that are recovering from substance abuse, educating members of the healthcare team, and patients regarding substance abuse. The Pharmacist will provide safe, effective, appropriate and cost-efficient medication therapy to patients suffering from substance use disorders (SUD). Under the direction of the Pharmacy Clinical Manager the Substance Use Disorder Pharmacist will assist in the planning, organizing and monitoring of pharmacy services for Bayhealth as related to SUD.
1. Provide comprehensive review of medications patients are using prior to presenting to Bayhealth; this includes utilization of Prescription Monitoring Program to assess narcotic usage. Provide patient specific regimens to avoid SUD in patients starting opioids and in patients with a known or suspected history of SUD; this includes patient and caregiver education. 2.
Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data. Utilize laboratory screening for opioids and illicit substances to help formulate a plan to treat patients with substance use disorder. 3. Initiate, titrate, and manage evidence-based pharmacotherapy for SUD including follow-up and monitoring for withdrawal symptoms and adverse effects.
Use shared decision-making with the healthcare team to promote collaborative, evidence-based, patient-centered care with treatment goals. 4. Conduct staff development programs and provide educational services to pharmacy staff, nursing staff, patient care services, physicians, and other healthcare professionals regarding SUD and current treatment options. 5. Serve as a subject matter expert in SUD and complete SUD related consults as requested by the medical staff. 6.
Develop, enforce, and maintain policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate medication therapy use for patients with SUD. Review and evaluate requests for non-formulary and restricted medications used to treat SUD. 7. Establish relationships with local, state, or national organizations to stay informed about community resources. 8.
Working with Social Workers, serve as a liaison between recovery centers and Bayhealth to establish treatment plans and continuity of care. Refer willing patients to recovery centers so treatment may be continued after discharge. 9. Develop a comprehensive treatment plan for patients experiencing acute withdrawal either in the Emergency Departments or in patients who have been admitted. 10.
Modify existing treatment plans to facilitate patients being downgraded from an ICU-level of care to Medical/Surgical Units or discharge. 11. All other duties as assigned within the scope and range of

Bachelor Degree Pharmacy Credential(s): Licensed Pharmacist, Certified Pharmacist in Substance Use Disorders Within 1Year Or Certification in Pain Management from the Board of Pharmacy Specialties within one year of hire.
