Job Details Job Duties:
Ensures patient care coordination plans and activities comply with established policies and procedures, standards of care, regulatory requirements, budgetary parameters and strategic objectives.
Supports and fosters a commitment to continuous quality improvement by providing a coordinated system for evaluating patient care outcomes, quality measures, and Key Performance Indicator (KPI) performance.
In full support of System Aim, collaborates with staff and co-leaders to implement Care Coordination Model--roles, future state standard operating procedures (SOPs), SNAP and Escalation Huddles, and Operational Rounding.
Directs all aspects of the Care Coordination Model to improve the Length of Stay performance and patient throughput of the organization, to include the following: Throughput and Patient Placement, Administrative Care Management for Revenue Cycle, Care Coordination Center Operations and Leadership, Organizational Metrics/Measures of Success

Bachelor's Degree in Nursing required; Masters in Healthcare preferred

Posted Salary Range USD $69.00 - USD $78.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
