We are seeking a highly skilled and motivated Licensed Practical Nurse (LPN) to join our busy Surgery Practice. The LPN will be responsible for providing exceptional patient care by managing and coordinating various aspects of the patient's surgical journey, ensuring safety, quality, and adherence to schedules. This role requires the LPN to demonstrate advanced clinical skills, excellent organizational abilities, and a strong focus on patient advocacy Responsibilities 1.
Patient Navigation: Serve as a primary point of contact for patients throughout their surgical experience, guiding them through pre-appointment, office visits, hospitalization, and post-discharge phases. Provide comprehensive education and support to patients and their families. 2. Pre-Admission Testing (PAT) Management: Coordinate and schedule pre-admission testing procedures, ensuring all necessary tests and evaluations are completed accurately and on time.
Collaborate with healthcare providers and ancillary departments to streamline the PAT process. 3. Safety and Quality Assurance: Uphold strict safety protocols and ensure compliance with regulatory standards. Maintain a clean and safe environment for patients and staff.
Monitor and report any potential risks or incidents and actively participate in quality improvement initiatives. 4. Care Across Episode of Illness: Provide holistic patient care, recognizing the patient's needs across different stages of their illness or surgical procedure. Develop and implement individualized care plans, considering the patient's physical, emotional, and psychosocial well-being. 5.
Outcome Development: Collaborate with the healthcare team to establish measurable and individualized expected outcomes for patients. Continuously evaluate the effectiveness of the care plan and modify as necessary to achieve optimal patient outcomes. 6. Independent Organization and Evaluation of Care: Independently manage patient care by prioritizing tasks, coordinating appointments, and ensuring efficient communication among team members.
Regularly assess and reassess the patient's condition, promptly addressing any changes or concerns. 7. Patient Care
Take responsibility for the overall patient care experience, ensuring that patients receive compassionate and personalized care. Anticipate and address patient needs, providing emotional support and comfort during their surgical journey. 8. Room and Triage Patients 9.

- Previous experience working in a surgical practice or similar setting is preferred.
- Familiarity with surgical procedures, pre-admission testing, and post-operative care is highly desirable. 3. Skills and Abilities:

- Strong organizational and multitasking abilities to manage multiple patient cases simultaneously.
- Exceptional interpersonal and communication skills to effectively interact with patients, families, and the healthcare team.
- Ability to work independently, make sound judgments, and prioritize tasks in a fast-paced environment.
