Licensed Practical Nurses (LPNs) provide professional nursing care for assigned residents in accordance with the State Board of Nursing Practice Act, standards of practice, and professional performance guidelines. The LPN is responsible for assessment, diagnosis, outcomes identification, planning, implementation, and evaluation for residents served. The LPN is an active participant in decisions related to resident care and nursing practice and serves as an educator, advocate, and team player.
This role requires demonstrated competency in quality care, process improvement, resident and employee safety, collaboration, and professional development. The LPN is responsible for planning, organizing, directing, and evaluating resident care provided by Certified Nursing Assistants (CNAs).
* Ensures the timely assessment of the health status and needs of assigned residents. * Completes all documentation prior to the end of the shift. * Conducts reassessments on assigned residents and documents reassessments per unit policy and when a resident’s condition changes. * Initiates orders as incorporated into the plan of care. * Completes nursing interventions and physician’s orders in a timely manner during shift. * Accurately interprets, processes, and transcribes orders for medications, treatments, procedures, laboratory work, and other diagnostic services. * Follows Nursing Practice Guidelines and policies related to medication administration and IV therapy. * Oversees care on assigned residents to ensure quality and consistency of care delivery. * Correctly and accurately recognizes respiratory or cardiac arrest and immediately begins CPR and activates emergency requests for assistance (Rapid Response/Code Blue). * Demonstrates efficient, effective, and prompt responses to any unit/resident crisis situation. * With RN guidance and supervision, assesses resident’s and/or families understanding of disease process, treatment plans, and preventive measures; provides education and documents understanding. * Demonstrates accountability and responsibility for clinical decisions and assigned duties. * Provides care with respect for human dignity and the uniqueness of the resident without discrimination based on socioeconomic status, personal attributes, or health conditions. * Safeguards the residents and the public when health care and safety are affected by reporting incompetent, unethical, or illegal practices. * Completes Occurrence Reports for any event that deviates from standard practice. * Maintains an environment conducive to high quality nursing care. * Follows Dress Code specific to Nursing role as outlined in System policy * Does not use personal devices or cell phones during work hours.
Only hospital approved communication devices are approved for use during work hours. * Transports residents safely and as required by the residents’ level of care. * Rotates and flexes with a positive attitude to alternate wings as necessary to provide adequate staffing to meet the needs of resident care. * Accepts responsibility to establish and maintain conditions conductive to high quality nursing care. * Conducts routine rounding to address resident and family needs. * Demonstrates basic computer competency, including use of email and required systems; reviews organizational email at least weekly. * Notifies physicians and residents representative of significant changes in condition, including critical results, falls, or injuries. * Communicates resident status clearly and accurately during handoff and with interdisciplinary team members. * Attends at least 80% of monthly staff meetings * Keeps abreast of pertinent federal, and state regulations and laws and Tift Regional Health System, Inc.
(“TRHS”) policies as they presently exist and as they change or are modified. * Understands and adheres to: TRHS’ compliance standards as they appear in TRHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy; and HIPAA and TRHS policies regarding privacy and security of protected health information. * Demonstrates the ability to perform tasks that meet the age-specific requirements of the persons, patients, vendors, and staff that the employee is charged to interact with as required by the position. * Offers suggestions on ways to improve operations of department and reduce costs. * Attends all mandatory education programs. * Improves self-knowledge through voluntarily attending continuing education/certification classes. * Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements. * Cross-trains in order to better assist co-workers and to provide maximum efficiency in the department. * Volunteers/participates on hospital committees, functions, and department projects. * Manages resources effectively. * Reports equipment in need of repair in order to extend life of equipment and removes malfunctioning equipment out of service with timely reporting to the appropriate personnel. * Makes good use of time so as to not create needless overtime.
