SWHR Registered Nurse ASN

DEPARTMENT: SOUTHWELL HEALTH AND REHABILITATION F ACILITY: Southwell Medical WORK TYPE: Full Time S HIFT: Nighttime

Registered Nurses provide professional nursing care for assigned residents in accordance with the State Board of Nursing Practice Act, standards of practice, and professional performance guidelines. The RN is responsible for assessment, diagnosis, outcomes identification, planning, implementation, and evaluation for residents care. The RN actively participants in decisions related to resident care and nursing practice.

They serve as educator, colleague, researcher, advocate, and leader. They must demonstrate competency in providing quality care, process improvement, resident and employee safety, collaboration, consultation, and professional development. The RN assumes responsibility for planning, organizing, directing, and evaluating resident care provided by Licensed Practical Nurses, Certified Nursing Assistants (CNAs), and ancillary personnel in accordance with scope of practice and facility policies.

* Ensures the timely assessment of the health status and needs of assigned residents. * Completes all required documentation prior to the end of shift. * Conducts and documents reassessments on assigned residents in the electronic medical record. * Initiates orders as incorporated into the plan of care. * Completes nursing interventions and physician’s orders in a timely manner during the shift. * Accurately interprets, processes, requests, and transcribes orders for medications, treatments, procedures, laboratory work, and other diagnostic procedures and consults. * Communicates resident condition clearly and accurately during shift report, to physicians, and to interdisciplinary team members using appropriate handoff methods, with proper documentation. * Adheres to SWHR policies, standards of Nursing Care, and Nursing Practice Guidelines. * Oversees care for assigned residents to ensure quality and continuity of care. * Recognizes respiratory or cardiac arrest and immediately begins CPR and activates emergency requests for assistance (Rapid Response/Code Blue). * Demonstrates efficient, effective, and prompt responses to any unit/resident crisis situation. * Assesses resident and family understanding of disease processes, treatments, and preventive measures; provides education and documents understanding. * Demonstrates accountability for individualized clinical judgment and actions.

Demonstrates dependability and accountability in all assigned tasks and responsibilities. * Provides care service with respect for human dignity and the uniqueness of the resident without discrimination based on socioeconomic status, personal attributes, or health conditions. * Safeguards the residents and the public when health care and safety are affected by reporting incompetent, unethical, or illegal practices. * Completes Occurrence Reports for any deviation from standard practice. * Attends at least 80% of monthly staff meetings. * Follows Dress Code specific to Nursing as outlined in the system policy. * Does not use personal devices or cell phones during work hours, only hospital-approved communication devices are approved for use during work hours. * Transports residents safely as required by the resident's level of care. * Maintains compliance with policies related to information release, confidentiality, resident rights, chart access, and advance directives. * Adheres to all safety, infection control, restraint, fire safety, and equipment handling policies. * Recognizes the resident as the authority in their care and incorporates resident and family preferences into care planning. * Includes resident, family, and other healthcare providers in collecting and prioritizing PHI. * Notifies physician and resident’s representatives of significant changes in resident status, including Lab or test results, and falls. * Makes residents and family rounds to assist with prioritization and assuring needs met, quality care provided and concerns are addressed. * Advocates for appropriate and responsible use of interventions to minimize unnecessary treatment and resident discomfort. * Coordinates care with residents, families, and the interdisciplinary team. * Reports resident or employee injuries, unresolved complaints, unexpected/unusual situations, and near misses to nursing leadership and documents appropriately. * Maintains awareness of current professional trends and policies and procedures impacting resident care. * Demonstrates basic computer competency, including use of email and clinical systems; reviews organizational email at least weekly. * Reviews and interprets clinical data, including trends in vital signs and pain, and understands resident diagnoses and medical history. * Conducts rounding to meet resident and their family’s needs. * Maintains an environment conducive to high quality nursing care. * Rotates and flexes with a positive attitude to alternate wings as necessary to provide adequate staffing to meet the needs of resident care. * Evaluates resident progress toward goals and revises the plan of care as appropriate. * Keeps abreast of pertinent federal, and state regulations and laws and Tift Regional Health System, Inc.

(“TRHS”) policies as they presently exist and as they change or are modified. * Understands and adheres to: TRHS’ compliance standards as they appear in TRHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy; and HIPAA and TRHS policies regarding privacy and security of protected health information. * Demonstrates the ability to perform tasks that meet the age-specific requirements of the persons, patients, vendors, and staff that the employee is charged to interact with as required by the position. * Offers suggestions on ways to improve operations of department and reduce costs. * Attends all mandatory education programs. * Improves self-knowledge through voluntarily attending continuing education/certification classes. * Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements. * Cross-trains in order to better assist co-workers and to provide maximum efficiency in the department. * Volunteers/participates on hospital committees, functions, and department projects. * Manages resources effectively. * Reports equipment in need of repair in order to extend life of equipment and removes malfunctioning equipment out of service with timely reporting to the appropriate personnel. * Makes good use of time so as to not create needless overtime.
