Description

Job OverviewFairview is looking for a System Professional Practice Nurse Lead supporting adult Critical Care areas at various locations in the system.

This is a full-time (1.0 FTE) position, working primarily Monday-Friday, typical business hours. Flexibility required, occasional evenings & weekends.

The System Professional Practice Nurse Lead (PPNL) demonstrates expert knowledge and skill in a nursing practice specialty, service line, and across the continuum of care for the system. The PPNL leads work, processes, and people to design, develop, implement, and evaluate innovative evidence-based interventions and best practices to optimize patient safety, healthcare quality, cost-effectiveness, and improved patient and staff experience.

The PPNL generates and evaluates practices through partnership with nurses and other members of the inter-professional team and demonstrates professional leadership in work at the micro-, meso- and macrosystem levels. Patient Care and Evaluation of Clinical PracticeEvaluates care of the specialty patient/population against professional standards and evidence. Designs, implements, and evaluates practice and outcomesAnalyzes proposed practice initiatives to establish impacts on safety, effectiveness, and efficiency.

Acquires and analyzes data from multiple sources to guide design of practice changes. Leverages the organization’s evidence-based practice model to identify the highest quality evidence and disseminates findings from evidence-based practice & research to improve healthcare outcomes. Demonstrates sensitivity to diverse organizational cultures and populations, including patients and providers.

Collaborates in the development of nursing guidelines, performance standards, policies & procedures. Provides leadership and promotes the organizational professional governance model for the advancement and autonomy of nursing practice excellence in the organization. ConsultationEmploys consultative leadership skills within and among professional teams.

Creates an environment to improve capacity for learning and adaptation to change. Provides consultation in development of patient/family education materials. Communicates findings to key stakeholders and follows up with others to ensure closed loop communication.

Provides expert consultation regarding nursing licensure and scope of practice. Systems LeadershipPromotes competency and excellence in professional and clinical practice and collaborative working relationships across the continuum of care. Participates in and/or leads the development and review of policies and procedures across the continuum of care.

Performs assessments and designs plans to meet regulatory requirements related to nursing practice. Performs and communicates system level assessments to identify variables that influence nursing practice and outcomes. Leads teams to improve and evaluate nursing sensitive indicators and other clinical quality metrics to improve clinical performance.

CollaborationBuilds effective teams by including key stakeholders within and across units, departments, service lines, populations, sites, and organization and conducts routine intermittent stakeholder assessments as part of the project management process. Collaborates with and supports the work of nursing practice councils and teams to improve patient care and interdisciplinary workflows. Uses evidence-based strategies for leading, communicating, and facilitating changes in clinician practice and team behaviors.

Demonstrate contributions and leadership for system improvements through intentional rounding, collaboration, and development at the micro-, meso-, and macro- levels. CoachingmSupports the clinical and professional development of colleagues through role modeling, may serve as preceptor and mentor. Participates in professional development and staff educational programs to support continual nurse competence.

Supports the Professional Practice Model through integration into daily work, role modeling, and mentorship of nurses across the system. ResearchAcquires, analyzes, plans, disseminates, and implements evidence-based practice into the clinical environment to promote best practices. Initiates, facilitates, and/or participates in research activities or quality improvement activities.
