✓Requirements
Licensure
✓Active and unrestricted license in the state of Colorado and ability to start the process of obtaining additional licensure prior to start date
Active and unrestricted license in the state of Washington preferred
Education
✓NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
Qualifications
✓Ability to gain a collaborative practice agreement, if applicable in your state
✓All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy
Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs preferred
Experience working in a nursing home, or with seniors in an acute care facility preferred
Demonstrated understanding of Geriatrics, Chronic Illness, and acute disease management preferred
Demonstrated understanding of Advanced Illness and end of life discussions preferred
Proven ability to develop and maintain positive customer relationships preferred
Proven adaptability to change preferred

About the role

The Telehealth Urgent Care program is a comprehensive integrated care delivery program. The National On Call advanced practice clinician (APC) is responsible for providing telephonic/telehealth care and direction to patients, caregivers and facility staff providing 24/7 coverage including holidays.
