Participate in daily interdisciplinary team meetings, coordinate follow-up appointments, facilitate Adult Recovery Team meetings, and support urgent patient needs.
Build Strong Relationships:Maintain regular communication with patients, families, and treatment professionals, ensuring everyone involved in the recovery process stays informed and supported. Champion Patient Engagement:Encourage patients to actively participate in their treatment journey, including group therapy, treatment planning, and discharge planning activities. Empower Recovery:Connect patients with valuable community resources and support services that promote long-term wellness and successful recovery after discharge.
Ensure Compliance and Quality Care:Adhere to program guidelines, treatment protocols, and HIPAA requirements while supporting the highest standards of patient confidentiality and care. Advocate for Patients:Complete excellent documentation to support patients in an appropriate length of stay and acceptance to referral Level of Care. Masters or Doctoral degree in Social Work, Psychology, Professional Counseling or related degree.
Experience coordinating care, familiarity with community resources, and ability to be self-organizing. Experience in the field of Behavioral Health for at least 2 years is preferred, does not have to be obtained post-Master's Licensure/Certification: No license is required MINIMUM
Must be certified in CPR upon hire and bi-annually thereafter and must possess or obtain and be able to perform Handle With Care within 30 days of hire and re-certify as needed. Must complete all required mandatory in-services annually. Must possess an Arizona Fingerprint Clearance Card or the ability to obtain.
