The Trauma Outreach Coordinator provides a leadership role in trauma prevention outreach with an internal and external focus utilizing the Trauma Registry to identify and address institutional and community needs.
Develops, implements, and monitors a comprehensive injury prevention program suited to all ages to meet the needs of patients, families, staff, and the community. Collaborates with local, regional, state, and national organizations in the development and implementation of trauma outreach. Responsible for the overall coordination, organization, implementation, evaluation and tracking of the Trauma Injury Prevention and Outreach Program Activities.
Serves on and/or facilitates committees, councils, and task forces as assigned. Provides a coordinated care experience for patients/families throughout the continuum of care to ensure that optimal physical, psychological and functional outcomes are met, to manage. Evaluates programs and presentations to efficacy, efficiency, quality of presentation and appropriateness of the information including feedback from requesting agencies.
Assists in performance improvement activities as required. PHYSICAL AND SENSORY
Sit up to 4 hours per day; 2 hours at a time. Stand for up to 6 hours per day, 3 hours at a time. Walk 6 hours per day, 20 minutes at a time.
Frequently use hands and fingers. Occasionally lift, carry and push objects up to 75 pounds. Occasionally transport patients weighing up to 250 pounds via wheelchair, bed, and/or stretcher.
Frequently stoop and bend. Frequently reach above shoulder level. Must be able to perceive attributes of an object through touch.
Seeing as it relates to normal near and far vision, and hearing as it relates to both high and low frequencies.
Bachelors degree required in related field. TRAINING AND
