Bachelor’s degree strongly preferred.
Qualifications
✓Valid driver's license and current automobile liability insurance.
✓Minimum total of two (2) to three (3) years nursing experience in a home care environment.
✓Minimum of two (2) years clinical nursing experience in a medical/surgical or acute care environment.
✓Shows initiative and good judgment.
✓Excellent communications and organizational skills required.
✓Must be able to work independently and complete assignments with minimal supervision.
✓Must be able to coordinate prioritize and complete tasks simultaneously in a timely manner.
✓Must be customer-service oriented and able to blend strong clinical knowledge with excellent problem-solving and organizational skills.
✓Excellent interpersonal skills required.
✓Must be able to perform duties effectively in stressful emergency situations while maintaining a professional demeanor.

About the role

The LPN Triage Home Health assists the clinical management team in providing day-to-day clinical triage coverage and project development. They serve as a liaison with clinical staff, physicians and patients by assisting in the triage of clinical concerns, providing guidance to staff and patients, assisting with physician orders and speaking with primary care physicians and referral sources.
