Great opportunity for a Nurse Practitioner or Physician Assistant to join our Team at Koke Mill Family Medicine, located in Sprinfield, IL! This is a Family Medicine Advanced Practice Provider position, responsible for treating patients of all ages across the lifespan (pediatric through geriatric). This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients.

Licensure/Certification/Registry: - One of the following is required: - Must possess and maintain licensing as an APRN as required by Illinois State Law; - Must possess and maintain licensing as a PA as required by Illinois State law - Appropriate certification as a APRN or PA, as determined by department. - Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire). - Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required. - Valid CPR certification required. - ACLS where required.
