The Wellness Program Facilitator is responsible for assessing patients based on their medical history for admission into Wellness programs, including GlucoseZone, Wellness for Life, Live Well and others. Assists in the development of exercise prescriptions and protocols and supervises exercise therapy. Works with the Griffin Employee Age-Less Program to offer employees fitness center options, profiles, and program recommendations.
The Intake Coordinator is responsible for the first formal communication with patients who have been referred and scheduled by the primary care, cardiology, or endocrinology practices. Manages calibration, maintenance and use of exercise equipment and education of patients in equipment concepts. Support Clinical Wellness Nurse with outreach efforts.
