You’ll need
Certifications
✓Full time, first shift, Monday- Friday, some days/hours could go into evening hours depending on availablilty to clients needs.
Education
Minimum Bachelor’s degree from an accredited university preferred.
Qualifications
✓Complete State Wraparound plan training within 18 months of hire.
✓A minimum of 2 or more years of one or combination of the following experience
✓Case Management
✓Skill Building
✓Child Welfare
✓Juvenile Justice
✓Education in a K-12 school setting
✓Must hold a valid state issued driver's license.
About the role
The Wraparound Facilitator works in full partnership with child and family team members to develop a plan of care, oversees the implementation of the plan, identifies providers of services or family-based resources, assists in facilitating county consortium meetings, and monitors services for participants.
