Monday–Friday | 8:30 AM–5:00 PM📞 Call: None👥 Patient Volume: Approximately 12–16 patients per day💻 EMR: EPIC (HealthConnect)ResponsibilitiesEvaluate and manage adult patients within 7 days of hospital discharge. Conduct in-person, telehealth, and telephone follow-up visits. Participate in a new transitional care program designed to improve post-discharge outcomes.
